ABSTRACT

Objective To evaluate the clinical efficacy and safety of simultaneous integrated boost intensity-modulated radiotherapy (SIB-IMRT) to the tumor center in the treatment of bulky cervical cancer with massive bleeding in the first course radiotherapy.Methods Twenty-one cases with bulky cervical cancer complicated with massive vaginal bleeding were enrolled.At the first three times of external irradiation,a high dose radiotherapy (15 Gy/3 fractions) was delivered to the tumor center (the region retracted 2 cm from the periphery of cervical mass),followed by conventional irradiation (2 Gy/fraction) in the posterior course.Conventional dose irradiation (46 Gy/23 fractions) was given to the tumor periphery and pelvic lymphatic drainage area throughout the whole course.Concurrent chemotherapy by cisplatin at a dose of 25 mg/m2 was delivered weekly.After the external irradiation,intracavitary radiotherapy was given (20 Gy/4 fractions).Results Within 24 h after the first course radiotherapy,the volume of vaginal bleeding was significantly decreased by 50% and the bleeding was almost stopped within one week.The hemostasis rate was 100%.Conclusions SIB-IMRT into the center of bulky cervical cancer is an efficacious treatment of massive vaginal bleeding.

ABSTRACT

Objective To investigate the value of urodynamic in treating those patients with benign prostatic hyperplasia complicated with bladder dysfunction.Methods 130 patients with benign prostatic hyperplasia were retrospectively reviewed between December 2015 and December 2016.The exclusive criteria included nervous system diseases,moderate to severe diabetes,moderate to severe hypertension,urethral stricture,bladder stones,prostate cancer,etc.The patient's age is ranged 51 to 92,mean 72 years old.The previous history of disease ranged from 3 months to 15 years,mean 4 years.All patients accepted the urodynamic examination.Different treatments were considered according to urodynamic studies.Results According to the classification of bladder outlet obstruction,130 cases were classified as bladder outlet obstruction in 109 cases,bladder suspicious obstruction in 15 cases,no bladder obstruction in 6 cases.According to the classification of bladder function,130 cases were divided into unstable bladder in 6 cases,low compliance bladder in 7 cases,bladder atony in 15 cases,bladder without tension in 3 cases,normal bladder function in 99 cases.3 cases without bladder tension,including BOO in 2 cases,suspected BOO in 1 case,were accepted the cystostomy.15 cases with less detrusor contraction,including BOO in 4 cases,suspected BOO in 9 cases,were accepted the cystostomy and TURP.Conservative treatment was performed in other 2 cases without obstruction.In 6 cases with unstable bladder,2 cases without obstruction accepted the conservative treatment,4 cases with suspected BOO accepted the TURP.In 7 patients with low compliance bladder,2 cases without obstruction accepted the conservative treatment,5 cases with suspected BOO accepted the TURP.The duration of following up was 3-6 months.The symptoms of the patients in this group improved significantly during following up.Conclusion For patients with benign prostatic hyperplasia complicated with bladder dysfunction,the results of urodynamics can help to choose the right treatment and improve effect of treatment.

ABSTRACT

Objective:To investigate hypoxia location in condylar cartilage in the early growth stage of rats.Methods:40 Sprague-Dawley rats were breastfed from 1 4 d to 21 d of age.1 0 rats were sacrificed at 1 2,24,48 and 96 h respectively after initiation of normal food at 21 d of age.The rats were administered pimonidazole hydrochloride (HP-1 )at a dose of 60 mg/kg by intraperitoneal injection 2 h before sacrifice.The expression of HP-1 in the whole condylar cartilage was detected by immunohistochemical staining.Results:HP-1 was mainly expressed in the chondrocytes of the fibrous and proliferative layer of cartilage,primarily concentrated in the weight-bearing area of joint-anterior aspect of the condyle and posterior aspect of the articular eminence at all time points.The highest expres-sion was observed at 24 h after initiation of normal food (P <0.01 ).Conclusion:In the early growth stage of rats,dietary loading may directly induce hypoxia in uper layer of condylar cartilage,the hyoxia level may change with time of dietary loading.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the different expressions of ER and ER gene status between primary and relapsed/metastatic lesions and their clinical significance.</p><p><b>METHODS</b>ER and ER gene status of primary and relapse/metastatic breast cancer masked in 70 metastatic breast cancer patients were assessed by determination of methylation status by immunohistochemistry (IHC) and methylation specific polymerase chain reaction (MSP), respectively.</p><p><b>RESULTS</b>Positive rate of ER in the primary breast cancers was 64.3%, and in the relapse/metastatic lesions was 41.4% (P < 0.05). There were six patients whose positive ER status was changed to negative, among them the ER gene status was changed from demethylation to hypermethylation in four cases. Another four patients with negative ER status changed to positive, and their ER gene hypermethylation changed to ER demethylation status.</p><p><b>CONCLUSIONS</b>The discordance of ER expression status in primary and relapse/metastatic lesions of breast cancer might be related to DNA methylation status.</p>

ABSTRACT

<p><b>BACKGROUND</b>The multilineage differentiation potential ability of bone marrow stromal cells (BMSCs) showed great potential in tissue engineering, while vascular endothelial growth factor 165 (VEGF165) promotes vasculogenesis and further promotes tissue regeneration. This study aimed to assess the ability of rat BMSCs expressing human VEGF A165 (hVEGF165) to promote tissue repair in rat model of radiation-induced injury.</p><p><b>METHODS</b>Rat BMSCs were isolated from the tibia. Plasmid DNA expressing hVEGF165 was stably transfected into BMSCs using liposomes. The right hindlimb muscle of 40 rats was irradiated using a (60)Co γ source (total dose 30 Gy). The animals were divided into four groups (n = 10): not injected with BMSCs (control; group 1) or intramuscularly injected two times (once in 2 weeks) with pcDNA(TM)3.1-transfected BMSCs (group 2), untransfected BMSCs (group 3), or hVEGF165-transfected BMSCs (group 4). Angiography was performed 1 week after the last injection of BMSCs; samples of the hindlimb muscle were subjected to transmission electron microscopy, ultrastructural analysis, reverse transcription-PCR (RT-PCR), Western blotting, and immunohistochemistry.</p><p><b>RESULTS</b>Rat BMSCs with multipotent differentiation capacity were isolated. hVEGF165-transfected BMSCs overexpressed hVEGF165 mRNA and protein. Injection of BMSCs (groups 2-4) increased the average vessel number, density, diameter, and cross-sectional area; mRNA expression of the myogenic markers including myoblast determination protein, myogenin, and a-smooth muscle actin; and CD31 protein expression; and promoted the repair of blood vessels and myofibers after radiation-induced injury compared to group 1; each of these parameters and hVEGF165 mRNA or protein expression were markedly improved in rats injected with hVEGF165-transfected BMSCs compared to groups 2 and 3.</p><p><b>CONCLUSIONS</b>BMSCs expressing hVEGF165 enhanced the repair of radiation-induced tissue injury by promoting vasculogenesis and muscle fiber regeneration. BMSCs expressing hVEGF165 may have potential clinical applications.</p>

ABSTRACT

Objective To explore the clinical application and safety of treatment of percutaneous radioactive 125I seed implantation treatment in lung metastases under CT guidance.Methods Twenty-seven lung metastatic malignancy cases (67 nodules) were studied.Eighteen cases (46 nodules) were hepatic cancer,4 cases (9 nodules) were prostate cancer,and 5 cases (12 nodules) were breast carcinoma.Diameters of lung nodules ranged from 0.5 cm to 4.0 cm with an average diameter of 2.1 cm.125I seeds were embedded under CT guidance,using 2 to 33 particles/nodule with an activity of 18.5 to 29.6 MBq/grain for each particle.Tumor matched peripheral dose was 90-120 Gy.Postoperative validation and quality evaluation followed.Results Four months later,24 nodules showed CR,30 showed PR,5 showed NC and 8 showed PD.The total effective rate was 80.6% (54/67).In the course of treatment,11 patients had pneumothorax,3 had heavy lung compression and 4 had hemoptysis.All conditions were improved by pleural puncture or under close follow-up observation.Conclusion 125I seed implantation is an effective and safe technique in treatment of metastatic lung tumors under CT guidance.

ABSTRACT

Objective To evaluate the effect of replication protein A (RPA) gene suppression on the radiosensitivity of esophageal cancer cells (TE-1R ) and underlying mechanism. Methods A radioresistant human esophageal cancer cell line TE-1 R was screened out by fractionated irradiation to TE-1 cells,then siRPA1 or siRPa2 was transfected to TE-1R cells.The untransfected (Con) group and nonsense siRNA transfected (NC) group were set as control groups.The survival was measured with colony-forming assay and the cell cycle distribution was measured with flow eytometry.Results Compared with the Con and NC groups,the protein expression of RPA1 and RPA2 decreased significantly 48 h after siRPA1 and siRPA2 transfection.The D0,Dq,and SF2 values reduced from 2.09,1.70,0.85 in NC group to 1.67,0.71,0.44 and 1.82,0.89,0.51 in siRPAl and siRPA2 transfection groups,respectively.Accordingly,the sensitization enhancement ratios of Dq were 2.39 and 1.91,respectively.The G2/M arrest in siRPA1 and siRPA2 transfection groups increased from (18.701 3.14)% of NC group to (26.95 ± 3.96)% and (25.28 ± 2.74) % (t =2.827,2.853,P <0.05),respectively.Conclusions Knocking down of RPA1 or RPA2 genes can enhance the raidosensitivity of human esophageal cancer cells TE-1R,where the inhibition of radiation-induced sublethal damage repair may be involved.Accordingly,RPA may become a new target of radiosensitization in esophageal cancer.

ABSTRACT

ObjectiveObserve the efficacy, toxicity and the effection of telomerase activity of allicin combined with paclitaxel plus cisplatin and 5-Fu(TFP)chemotherapy for patients with advanced gastric carcinoma.MethodsFifty-four patients with advanced gastric carcinoma were randomly divided into two groups. A group was treated with allicin and TFP, B group was only treated with TFP. The chemotherapy was paclitaxel (135 mg/m2, dl, 8) plus cisplatin (75 mg/m2, d2-4) followed by 5-Fu (500 mg/m2 d1-5). Allicin was used in 60 days(3 times every day,once 20 mg).Telomerase activity was performed by enzyme-linked immunosorbent assay. ResultsThe total efficiency rate was 40.7 % (11/27) in A group and 33.3 % (9/27) in B group.There was no statistical difference between the two groups(~ = 0.079,P =0.779).There was significant difference in nausea and vomit (P =0.043), and the incidence of A group was 77.8 % (21/27) which was lower than that of B group[92.6 %(25/27)].There was statistical difference in telomerase activity between the two groups(P =0.000).ConclusionAllicin could reduce the toxicity of chemotherapy and inhibit the telomerase activity in advanced gastric carcinoma.

ABSTRACT

A rapid and specific high performance liquid chromatography-mass spectrometric method was developed for determination of magnoflorine in Rhizoma Coptidis and Cortex Phellodendri Chinensis. Samples were extracted by methanol. Agilent Eclipse XDB-C18 ODS column (4.6 mm x 150 mm, 5 microm) was used, and mobile phase was methanol-water (60:40) at a flow rate of 0.8 mL x min(-1). Electrospray ionization model (ESI), and MRM model were used for quantification. The linear range of magnoflorine was 4.352-2720 microg x L(-1). The average recovery was above 98%. The method is simple, sensitive and accurate, it can be used for determination of magnoflorine in Rhizoma Coptidis and Cortex Phellodendri Chinensis.

ABSTRACT

Objective To explore a novel improved method of simplified fluid-supplement therapy during the diuresis stage after renal transplantation.Method We adopted an improved fluid-supplement therapy to treat 26 postoperative patients of heterologous kidney transplantation after returning Liger's liquid, 500ml of 5% glucose-salt liquid, and 500ml liger's Compound back to linical wards by means of replenishing 500ml of sodium lactate sodium chloride injection in turn for fluid supplement. In addition, the patients were given originally 10~20ml of 10% calcium glubionate within 12 hours after their operations. Results Based on the results of both blood and urine determination at the first day aftert kidney transplantation, we discovered 2 patients slightly decreased sodium in blood, one displayed hypokalemia,2 patients showed slightly hyperkalemia value than normal referance level. Meanwhile, all patients manifested blood chlorate value between 101?4. Besides, another 7 patients had slightly increased blood suger value. However, no hypernatremia, hypocalcia or hypomagnesemia were found. As a result, all specific gravity ratio value were from 1.010 to 1.015 aparting from hemobilinuria existed in some patients. Conclusion The succesessful use of the new fluid-supplement therapy could effectively avoid unnecessary repeated haemospasia analysis for mornitoring and adjusting the balance of electrolyte, which could remarkably simplify and faciliate the postoperative management of renal transplantation.

ABSTRACT

Objective To define the rational extent of radiation portal in adult patients with early-stage Hodgkin's disease (HD).Methods From Jan 1984 to Dec 1997,206 adult(≥15 year-old) HD patients with stage Ⅰ~Ⅱ lesions were admitted into the cancer hospital of Chimese Academy of Medical Sciences. 130 cases received radiotherapy alone (RT group). 76 cases radiotherapy combined with chemotherapy (R+C group). Involved-field (IF) was used in 7 patients (3.4%),mantle-field(MF) in 34 (16.5%) patients,subtotal nodal irradiation (STNI) in 140 (68.0%) and total nodal irradiation (TNI) in 25 (12.1%) patients. Kaplan-Meier method was used in survival analysis and log rank test for comparson.Results ①The overall 5-,10-year accumulated survival rates (ASR) were 85.1% and 73.2%. The 5-,10-year disease free survival rates (DFSR) were 68.0% and 63.6%, respectively.② The 5-year ASRs in the MF,STNI and TNI groups were 69.2%,93.3% and 94.4%,respectively (P<0.05);the 5-year DFSRs in these respective groups were 54.2%,79.2% and 79.9% (P<0.05).③ In R+C patients ,the 5-ASRs in the IF/MF and STNI/TNI groups were 75.7% and 90.6%(P<0.05); 5-year DFSRs were 43.1% and 73.3%(P<0.05).Conclusions The stage ⅠA patients with favouable prognostic factors can be cured by using mentle-field only,but patients in the other early-stages should be given subtotal nodal irradiatiion when radiotherapy is given alone. It is suggested that patients with unfavorable prognostic factors should receive radiotherapy combined with chemotherapy.
